Report to work rested, alert and fit to give your full attention to the job. Of course alcohol and drug use is prohibited on the premises as is the residual results of use. Any employee discovered under the influence of alcohol or drugs will be terminated. 2. All work-related injuries must be reported to your supervisor and Workbox Staffing immediately. 3. Report any unsafe conditions to your supervisor and Workbox Staffing. NO EMPLOYEE IS EVER REQUIRED to perform work that he or she believes is unsafe, or that he or she thinks is likely to cause injury or a health risk to themselves or others. Contact WorkBox immediately if you have any safety concerns your worksite supervisor cannot address. 4. Horseplay, ‘practical jokes,’ etc., are forbidden. Employees are required to work in an injury-free manner displaying accepted levels of behavior. Conduct that places the employee or others at risk, or which threatens or intimidates others, is forbidden. 5. You are responsible to keep your work area clean and safe. Maintain a general condition of good housekeeping in all work areas at all times. 6. Learn where fire extinguishers and first aid kits are located at each assignment. 7. At the time of your work assignment you will be instructed on wearing Personal Protective Equipment (PPE) if it is required for the assignment: Inspect PPE prior to each use. Do not use damaged PPE. You are required to maintain and keep PPE clean. Not wearing required PPE is cause for disciplinary action up to and including termination. 8. You must specifically be authorized by WorkBox and trained by your supervisor to operate the following equipment INCLUDING, but not limited to: Forklifts, Machines, Power Tools, Welders, and Cranes/ Hoist while on assignment. Contact WorkBox immediately if you are asked to do 9. When operating machines: do not wear loose clothing, ensure that long hair is tied- back, remove jewelry, and keep long sleeves either rolled up or pulled all the way down. 10. Never operate damaged or defective equipment. Turn the machine off and report it to your supervisor immediately. 11. Never tamper with, remove, or deactivate machine guards or controls designed to ensure safe operation. Never operate machinery without guards in place. 2 12. Never reach into a machine to clear a jam, clean inside the machine or for any other reason unless it has been properly locked out at the power source(s). 13. Do not remove any lock or tag from machinery. 14. Use proper lifting techniques and if an item is too heavy or awkward to lift, look for an alternative method and/or have another employee help you lift the item. 15. Learn safe work practices. When in doubt about performing a task safely, contact your supervisor for instructions and training. 16. Always perform your assigned tasks in a safe and proper manner; do not take shortcuts. The taking of shortcuts and the ignoring of established safety rules is a leading cause of employee injury. 3 HAZARD COMMUNICATION – RIGHT TO KNOW If your assignment or work site has or uses any hazardous chemicals you will get specific training on the handling, personal protective equipment required, and use of these chemicals at your job site. This training is a basic introduction to OSHA requirements. PURPOSE OF OSHA’S HAZARD COMMUNICATION STANDARD To ensure that employers and employees know about work hazards and how to protect themselves so that the incidence of illnesses and injuries due to hazardous chemicals is reduced. EMPLOYER RESPONSIBILITIES • Identify and list hazardous chemicals in their workplaces • Obtain Safety Data Sheets (SDSs) and labels for each hazardous chemical, if not provided by the manufacturer, importer, or distributor • Implement a written HazCom program, including labels, SDSs, and employee training • Communicate hazard information to employees through labels, SDSs, and formal training programs WRITTEN HAZCOM PROGRAM REQUIREMENTS • Describes container labeling, SDSs, and employee training for each workplace • List of the hazardous chemicals • Make information regarding hazards and protective measures available to other employers onsite HOW MUST CHEMICALS BE LABELED? Each container of hazardous chemicals entering the workplace must be labeled or marked with: • Identity of the chemical • Appropriate hazard warnings • Name and address of the responsible party 4 SAMPLES OF LABELING AND HOW TO READ LABELS Chemical Name 0 = minimal hazard 1 = slight hazard HEALTH 0 2 = moderate hazard FLAMMABILITY 3 = serious hazard 0 4 = severe hazard PHYSICAL HAZARD 0 PERSONAL PROTECTION 0 SPECIAL HAZARD The Blue Section – Health Risks 4 The substance is a severe health risk if the substance is not handled safely. Substances carrying a four in the blue section could cause death or irreversible injury. 3 The substance could cause serious temporary or irreversible injury. 2 The substance could cause temporary incapacitation. 1 The substance could cause irritation. 0 There is no health hazard. 5 The Red Section – Fire Risks 4 A flammable vapor or gas, which burns readily. 3 A flammable liquid or solid, which can be readily ignited. 2 The substance must be heated for ignition. 1 The substance must be preheated before ignition can occur. 0 There is no fire hazard. The Yellow Section – Reactivity Hazard 4 The substance is readily capable of detonation or explosive reaction. 3 The substance may detonate when exposed to heat or an ignition source. 2 The substance is readily capable of non-explosive reaction. 1 The substance may become unstable at high temperatures. 0 The substance is stable. The White Section – Special Hazards OX Oxidizer ACID Acid ALK Alkali COR Corrosive W Use no water Radioactive 6 PERSONAL PROTECTIVE EQUIPMENT 7 Safety Data Sheets (SDS) The location of SDS Sheets is noted on your employee assignment sheet and/or communicated to by your site supervisor at the start of your assignment. These sheets are prepared by the chemical manufacturer or importer and describe: • Physical hazards, such as fire and explosion • Health hazards, such as signs of exposure • Routes of exposure • Precautions for safe handling and use • Emergency and first-aid procedures • Control measures • Must be in English and include information regarding the specific chemical identity and common names • Must provide information about the: • Physical and chemical characteristics • Health effects • Exposure limits • Carcinogenicity (cancer-causing) • Identification (name, address, and telephone number) of the organization responsible for preparing the sheet • Must be readily accessible to employees in their work area EMPLOYEE RESPONSIBILITY - SAFETY HANDLING OF CHEMICALS a) Do not handle chemicals that you are not trained to handle. b) A Safety Data Sheet (SDS) must be secured for all chemicals purchased or brought on site. You have a right to access SDSs – ask your supervisor. c) Follow all label and SDS instructions – including amount instructions d) Do not mix chemicals unless authorized to do so. e) Keep all chemicals in closed containers. 8 f) Store all flammable liquids in safety cabinets or safety cans. Never use flammable chemicals around ignition sources such as smokers, pilot lights, or arcing/sparking electrical equipment. g) Wear required Personal Protective Equipment and minimize contact with the chemical. h) Do not eat, drink, or smoke while using chemicals. And always wash your hands after handling chemicals. i) Do not attempt to clean up a hazardous substance unless you are properly trained and authorized. 9 EMERGENCY PROCEDURES 1. In the event of any serious injury or fire, call 911 unless specifically directed or trained by your site supervisor to use a different number or protocol. Send someone to the facility entrance to meet the Fire Department. If in doubt, call 911. 2. Should you discover a fire while on assignment, alert others in immediate danger and initiate facility-wide fire alarm. 3. When an evacuation signal is given for any reason, all employees should immediately turn off equipment, close doors, and evacuate to their designated evacuation areas. Attendance should be taken to account for all personnel. Stay together in the group until further instructions are received. 4. WorkBox does not expect their employees to fight fires at the workplace. Your first priority is your safety and the safety of others. Under no circumstances should you attempt to fight any fire which is larger than a waste basket, too hot, too smoky, or if you are concerned for your safety. 5. If you choose to use a fire extinguisher, remember PASS: • P = Pull (the safety pin) • A = Aim (at the base of the fire) • S = Squeeze (the lever) • S = Sweep (side to side) If you use a fire extinguisher, remember: • Stay low, • Keep yourself between the fire and an exit, • Do not turn your back on a fire, and • Immediately report the use of the extinguisher to your site supervisor and to your WorkBox supervisor.. BLOOD BORNE PATHOGENS The term ‘universal precautions’ means that you should assume that blood or other bodily fluids contain bloodborne pathogens that can cause disease and to protect yourself accordingly. Do not touch another person’s blood or any other bodily fluid during or following an incident. If you are trained to administer first aid, use gloves and other barriers that should be located with the first aid equipment. Always use protective personal equipment such as gloves if cleaning up blood or other bodily fluid and use a diluted bleach solution or other sanitizing agent. Dispose of PPE and cleaning towels in a secure way that won’t potentially infect others. If you think that you have been exposed to bodily fluid, notify your site supervisor and your WorkBox supervisor immediately. 10 11 PERSONAL PROTECTIVE EQUIPMENT (PPE) PPE includes all equipment designed to provide protection to the wearer from potential hazards to the eyes, face hands, head, feet, ears, and extremities. Eye/Face Protection – Equipment designed to provide protection to the face and eyes during exposure to such hazards as: flying particles, molten metal or sparks, liquid chemicals, acids or caustic liquids, or potentially injurious light radiation (i.e., lasers, welding etc.) Foot Protection – Equipment designed to provide protection to the feet and toes during exposure to situations with the potential for foot injuries such as: falling or rolling objects, chemical or liquid exposures, piercing objects through the sole or uppers, and/or where the employee’s feet are exposed to electrical hazards. Hand Protection – Equipment designed to provide protection to the hands during exposures to potential hazards such as sharp objects, abrasive surfaces, temperature extremes, and chemical contact. Hand protection is selected based upon the hazards and performance characteristics of the gloves. Head Protection – Equipment designed to provide protection to the head during exposures to potential hazards such as falling objects, striking against low hanging objects, or electrical hazards. Hearing Protection – Equipment designed to provide protection to an individual’s hearing during exposure to high noise levels. Respiratory Protection – Equipment designed to provide protection to the wearer from potential inhalation hazards such as: vapors, mist, particulates, and gases. EMPLOYEE RESPONSIBILITY 1. The employee assignment sheet outlines the basic safety equipment that is required to be worn when on assignment. 2. Additional PPE may be required when performing certain tasks. Your site supervisor is responsible for training you and showing you the proper use and wear of PPE. 3. You are expected to attend the required training. 4. You are also responsible for caring for, cleaning, storing and maintaining PPE as required. 5. Inform your site supervisor immediately of the need to repair or replace PPE. Also inform your WorkBox supervisor. 12 EYE PROTECTION What are some of the causes of eye injuries? • Dust and other flying particles, such as metal shavings or sawdust • Molten metal that might splash • Acids and other caustic liquid chemicals that might splash • Blood and other potentially infectious body fluids that might splash, spray, or splatter • Intense light such as those created by welding and lasers Safety Glasses • Used for moderate impact from particles produced from jobs such as: carpentry, woodworking, grinding, and machine operation. • Requires side shields. • Prescription glasses are not safety glasses. You are required to have other prescription safety glasses. Goggles • Used to protect eyes, eye sockets, and the facial area immediately by surrounding the eyes from impact, dust, and splashes. • Some goggles fit over corrective lenses. 13 EYE PROTECTION (CONTINUED) Welding Shield • Protects eyes and burns caused by infrared or intense radiant light. • Protects face and eyes from flying sparks, metal splatter, and slag chips produced during welding, brazing, soldering, and torch cutting. Laser Safety Goggles • Protects your eyes from intense concentration of light produced by lasers. • Used in conjunction with curtains. Face Shields • Protects the face from nuisance dust and potential splashes or sprays of hazardous liquids. • Does not protect employees from impact hazards. • Safety glasses should be worn underneath shield. 14 HEAD PROTECTION What are some of the causes of head injuries? • Falling objects • Bumping head against fixed objects, such as exposed pipes or beams • Contact with exposed electrical conductors Classes of Hard Hats Class G (formerly Class A) • General service (e.g., mining, building construction, shipbuilding, and manufacturing) • Good impact protection but limited voltage protection Class E (formerly Class B) • Electrical work • Protect against falling objects, high-voltage shock/burns Class C • Designed for comfort, offers limited protection • Protects heads that may bump against fixed objects, but does not protect against falling objects or electrical shock Hard Hats • Hard hats offer more protection than bump caps. They have an outer shell that is hard and durable to deflect potential head blows. Inside, hard harts have a lining that is shock absorbing. Also, some hard hats can provide protection from electrical shocks. Bump Hats • Bump caps are more lightweight than hard hats, making them more comfortable and proving to be less stressful on your neck, head, and back. • They are ideal in situations where it is not required to wear a hard hat, but injury could still occur. They are often used in warehouses and food processing plants. 15 FOOT PROTECTION What are some causes of foot injuries? • Heavy objects such as barrels or tools that might roll onto or fall on the employee’s feet • Sharp objects such as nails or spikes that might pierce the soles or uppers of ordinary shoes • Molten metal that might splash on feet • Hot or wet surfaces • Slippery surfaces Steel Toe Shoes/Boots • Safety shoes/boots that have steel reinforced toes in case something fall on your feet. • Commonly required by some employers and readily available at most shoe stores. Metatarsal Boots and Covers • Protects the upper foot (metatarsal bones) and toe area from hazards. A work boot with a built-in metatarsal guard will protect your upper foot and toe area from heavy falling objects- especially important for a workspace with heavy equipment, machinery, and containers. Rubber Sole/Non-slip/Closed Toe Footwear • You are required to wear rubber sole/non-slip/closed toe footwear on all assignments. Rubber Boots • To be used when working in slippery/wet conditions and while using chemicals. 16 HEARING PROTECTION When do you need hearing protections? • Noise can damage your hearing permanently whether at work, home, or during leisure activities. • If the noise is 85 decibels or more, you are required to use ear protection. • A general way to measure this is if you have to raise your voice to talk to the people standing next to you. Earplugs • Reduces noise levels to protect hearing. • These are convenient and disposable. Earmuffs • Easy to use and comfortable to wear. • Reduces noise levels to protect hearing. Canal Caps • Easy to use and comfortable to wear. • Reduces noise levels to protect hearing. • Hearing protection on a band that can be worn under the chin, over the head, or behind the neck. • These can be put on and taken off easily. 17 HAND PROTECTION Common Hand Injuries you need to guard against • Burns • Bruises • Abrasions • Cuts • Punctures • Fractures • Amputations • Chemical Exposure Metal Mesh Gloves or Kevlar Gloves These gloves are used to protect hands from accidental cuts and scratches. They are most commonly used by persons working with cutting tools or other sharp objects and materials. Fabric/Canvas Gloves Made of cotton or fabric blends and are generally used to improve grip when handling slippery objects. They also help insulate hands from mild heat or cold. These gloves do not provide a lot of protection against cuts or lacerations. Leather Gloves These gloves are used to guard against injuries from sparks or scraping against rough surfaces. They are also used in combination with an insulated liner when working with electricity. Aluminized Gloves Gloves made of aluminized fabric are designed to insulate hands from intense heat. These gloves are most commonly when working with molten materials. Chemical Resistant Gloves These gloves are made of rubber, neoprene, polyvinyl alcohol, or vinyl. The gloves protect from corrosives, oils, and solvents. You should consult your supervisor for the proper hand protection. 18 MACHINE GUARDING The machines you work with can contain moving parts with many hazards. Best safety practices involve the use of engineering controls and safe work procedures designed and developed to protect you when working with machines. This includes the use of machine guards. Machine guards safely contain and create a barrier of protection against the dangers. • Know the potential hazards associated with the machines you use • Understand the role of machine guards and their function • Follow safe work practices when working with machines • Inspect the machine guards on a regular basis • Report any missing or malfunctioning guard immediately NEVER REMOVE GUARDS As a machine operator, you are directly affected by machine related hazards. Personal protective equipment, engineering controls, and safe work practices are all important parts of a safe working environment. Machine guards are only one part of this formula. It is your responsibility to never remove or override these safeguards. A guard that is overridden or disregarded is the same as no guard at all. EXPOSURE TO ENERGIZED PARTS Contact with energized parts can cause electrical shock. The severity of the shock is determined by: the nominal voltage, the length of contact, the route of electricity through the body. EXPOSURE TO MOVING PARTS Nip points, pinch points, shear points and snag points are the cause of many finger and hand injuries including amputation when not properly respected. Motions such as rotating, reciprocating and transversing, become hazardous when clothing, jewelry or skin contact moving parts. Conveyor belts, gears and couplings are all examples of machine parts with hazardous motions. Actions occur at the point of operation and usually involve one of the three types of motions described earlier. Actions include: Cutting, Punching, Shearing, and Bending. PROJECTILES Projectiles from scrap material and foreign objects such as dropped tools can cause injury when working with machines. If an area with parts moving at high speeds is not properly guarded, a tool or item dropped or knocked into this area can become a dangerous projectile. 19 MACHINE GUARDING (Continued) CATEGORIES OF GUARDS There are a great number of different types of machine guards found in the work environment. All types of machine guards fall into one of three categories: Barrier guards, Distance guards and Physical guards. Fixed A fixed guard is a permanent fixture of the machine and does not depend on moving parts to function. It is generally constructed of sheet metal, screen, wire cloth, bars, plastic, or any other material that can withstand impacts and prolonged use. Interlocked When an interlocked guard is opened or removed, the tripping mechanism and/or power automatically shuts off, effectively stopping all moving parts. This prevents the machine from cycling or starting until the guard is back in place. An interlocked guard uses electrical, mechanical, hydraulic, or pneumatic power or any combination of these. Adjustable Adjustable guards provide a barrier that can be adjusted for various production operations. Self-adjusting A self-adjusting guard automatically adjusts for the size of the stock. Once the stock is removed, the guard returns to its normal resting position. It may be constructed of plastic, metal, or any other substantial material. CONFINED SPACE WorkBox does not allow temporary employees in permit-required confined space. If you believe that your job requires permit-required confined space entry, contact your WorkBox supervisor prior to undertaking the work. (Permit-required confined spaces are areas not meant for human occupancy, have limited means of entry/exit, and have electrical, chemical, thermal, atmosphere, or entrapment hazards). 20 LOCK OUT TAG OUT Prior to working on any machinery when guards are removed, every energy source (electrical, hydraulic, chemical, mechanical, etc.) must be deactivated, stored energy dissipated, and the control (for example, the circuit breaker) padlocked so the machine cannot be started without removing the lock. Never remove or tamper with a lockout performed by another employee or contractor. A lockout consists of a lock applied to a control such as a switch, breaker, or valve. A tag containing words such as “DANGER - DO NOT OPERATE” may also be used if the machine cannot be locked out for some reason (‘tagout’). If you see the lock, the tag, or both applied to an energy control device it means, “Keep your hands off.” If you are asked to clean, clear out a jam or maintain machinery that exposes you to moving or energized parts, you must be trained in the machine-specific lockout procedure and follow that procedure exactly. In no case should you put your hands or body inside a machine that has not been properly locked out at the energy source by you. PREVENTING SLIPS AND FALLS You can reduce the risk of slipping by: 1. Taking your time and paying attention to where you are going 2. Adjusting your stride to a pace that is suitable for the walking surface and the task you are doing 3. Walking with the feet pointing slightly outward 4. Making wide turns at corners 5. Wearing slip resistance shoes 6. Looking out for foreign substances on floors, such as: deposits of water, food, oil, sawdust, soap, or debris 7. Immediately reporting, cleaning up and/or posting warnings about the presence of water and other slipping hazards when you see them so others don’t slip. You can reduce the risk of tripping by: 1. Keeping standing and walking areas clear from clutter or obstructions. Notify a site supervisor if necessary. 2. Keeping flooring in good condition 3. Always using installed light sources that provide sufficient light for your tasks 4. Using a flashlight if you enter a dark room where there is no light 5. Ensuring that things you are carrying or pushing do not prevent you from seeing any obstructions, spills, etc. 21 Ladders and work at heights: WorkBox has height restrictions for temporary assignments. You should not climb ladders or work at heights that require your feet to be more than xx feet off the ground. 1. Inspect all ladders prior to each use 2. Ladders must be placed on secure footing 3. Only one person is allowed on a ladder at a time 4. Never stand on the top two steps of a stepladder 5. Always maintain 3-point contact when working on ladders 6. Never reach beyond arm length when working on a ladder 7. Never use metal ladders when working on or around electrical equipment or electrical wires Other work at heights could include lifts or raised work areas. You must be trained by your site supervisor in proper fall protection guidelines and provided with adequate fall protection whenever you are working more than six feet off the ground. Cranes/Hoist/Lifting Devices: 1. Inspect all cranes, hoists, and lifting devices (slings, hooks, etc.) prior to each use, and never use damaged equipment 2. Never walk under a load suspended from a hoist or crane 3. Keep all personnel clear of the ‘fall zone’ of the crane/hoist 4. Know the weight of material being lifted, and never overload a crane/hoist 5. Do not operate devices unless you have completed the required safety training 22 PROPER LIFTING TECHNIQUES Plan ahead before lifting. Knowing what you're doing and where you're going will prevent you from making awkward movements while holding something heavy. Clear a path, and if lifting something with another person, make sure both of you agree on the plan. Lift close to your body. You will be a stronger, and more stable lifter if the object is held close to your body rather than at the end of your reach. Make sure you have a firm hold on the object you are lifting, and keep it balanced close to your body. Feet shoulder width apart. A solid base of support is important while lifting. Holding your feet too close together will be unstable, too far apart will hinder movement. Keep the feet about shoulder width apart and take short steps. Bend your knees and keep your back straight. Practice the lifting motion before you lift the object, and think about your motion before you lift. Focus on keeping your spine straight--raise and lower to the ground by bending your knees. Tighten your stomach muscles. Tightening your abdominal muscles will hold your back in a good lifting position and will help prevent excessive force on the spine. Lift with your legs. Your legs are many times stronger than your back muscles--let your strength work in your favor. Again, lower to the ground by bending your knees, not your back. Keeping your eyes focused upwards helps to keep your back straight. If you're straining, get help. If an object is too heavy, or awkward in shape, make sure you have someone around who can help you lift. Wear a belt or back support. If you are lifting in your job or often at home, a back belt can help you maintain a better lifting posture. TIPS Never bend your back to pick something up. It's just not worth the damage that improper lifting technique can cause. Remember to use your leg muscles to help you lift properly. Hold the object close to your body. You are a much more stable lifter if you're not reaching for an object. Don't twist or bend. Face in the direction you are walking. If you need to turn, stop, turn in small steps, and then continue walking. Keep your eyes up. Looking slightly upwards will help you maintain a better position of the spine. 23 The Manufacturing Material Handler is responsible for supplying raw materials to production work cells and moving finished goods to proper warehouse bin location. This position also supports general warehouse functions such as loading and unloading trucks and moving items to proper location. Essential Functions and Responsibilities: * Safely utilizes a powered industrial truck to supply production cells with needed raw materials to meet production demands. * Shrink wraps, inspects, and delivers finished orders to proper warehouse bin locations.* Utilizes scanning system meticulously to track movement of product throughout different movements. * Notifies production supervisor or supply chain department of inventory discrepancies. *Follows bill of materials to correctly stage batching supplies for batch tenders. *Collaborates with packagers on correct raw materials needed to complete work orders. * Load and unload trucks as called upon by the shipping and receiving department. * Ensures clean work environment including sweeping and cleaning work areas. * Provides back-up assistance to other production positions as needed. * Complies with all safety, health, and regulatory compliance laws and policies. * Other duties/tasks as assigned by supervisor. Minimum Qualifications: * High school diploma or GED is preferred, or 3 months of related experience and/or training, or an equivalent combination of education and experience. * A demonstrated ability to read and follow formulas and instructions. * Must have ability to obtain powered industrial truck permit and safely operate a powered industrial truck. * This position required the physical capability to wear and safely utilize a respirator during certain essential functions. Working Conditions and/or Physical Demands: * Must regularly lift and/or move up to ten pounds, frequently lift and/or move up to fifty pounds, and occasionally lift and/or move more than one hundred pounds. * Frequently required to stand, walk, stoop, knee, and crouch.
